Dental, Vision and Hearing Flex plan features
This product has three distinct options with varying prices, benefits and coverage amounts. The flexible options will allow agents to tailor plans according to their client’s needs and budget. See the chart below.
- 15% multiple policy discount available
- No waiting period for dental services (preventative, basic, and major)
- Preventative services covered at 100%
- Same coverage for in-network or out-of-network providers
- Issue ages 18-89
- Whitening benefit available (options 1 and 2)
Credit for prior coverage available (option 2 only)
Credit for Prior Coverage immediately bumps the policyholder to the highest level of coinsurance for dental coverage only.
Requirements
- Prior coverage
– Must have been in effect for a minimum of 12 months
– Must not have been terminated for more than 60 days from the requested effective date at time of application
– Must have an annual benefit maximum of at least $1,000
– Must have covered Class C (Major) benefits - Necessary information (on company letterhead):
– Carrier name
– Policy number
– Start/End date of coverage
– Schedule of Benefits
– Primary Insured name
– Dependent name(s)

We will discontinue new sales of the existing Dental, Vision and Hearing/Plus plans
With the launch of the new Dental, Vision and Hearing Flex product, we will discontinue new sales of the existing Dental, Vision and Hearing and Dental, Vision and Hearing Plus products in the states where “Flex” is available. All current policies will remain in effect and commissions will continue to pay on those policies.
The last application signature and submission date for Dental, Vision and Hearing and Dental, Vision and Hearing Plus plans in the following states will be June 4, 2025: AL, AZ, CA, CO, DE, FL, GA, IA, ID, IL, KS, KY, MI, MO, MS, ND, NE, NJ, OK, OR, PA, RI, SC, SD, TN, TX, VT, WI, WV, WY
