
Get detailed insights on declined and rated LTC apps with new form
The Authorization to Release Underwriting information to Insurance Agent/Agency form (31368) is now available May 1 for standalone long-term care (LTC) applications administered by illumifin.
Why it’s important
If a traditional standalone LTC application is declined or rated and the client has signed the form 31368, the underwriter can provide you with additional details about the reason for that decision. The form is only applicable if an application is declined or rated.
How it works: If you would like to get more information about a standalone LTC app that’s declined or rated:
- Print the form from by clicking here, or search for Form 31368 on iPipeline’s FormsPipe system.
- Ask the client to complete and sign the form. Wet signature is required, and DocuSign cannot be accepted. Do not alter the form.
- Scan the completed form so you have a copy of it.
- Give the client the completed form.
- If submitting an eApp, upload the form along with any prescreen info as additional documentation (see image below).
- If submitting a paper application, submit the form with all other required forms.

Attaching documents to Thrivent’s eApp
Attachments can be added to the application only after the application is locked and signatures are captured. There is a space for up to four attachments and the total size cannot exceed 8MB.
NOTE: Only PDF files can be attached to an application.
- From the Left Navigation bar, select Attachments.
- Select an option from the drop-down list and click Attach PDF.
