The application and rates can be found on the Agent Resource Center.
- CO, OH and WY use state specific applications
- SD uses the generic application
- CO and OH use the same rates as MI, SC and TN
- SD has state specific rates
- WY uses the generic rates
This replaces Home Health Care Select in the launched states.
