
Mutual of Omaha remains dedicated to our policyholders by helping them safeguard what matters most and ensuring we are here when they need us. In line with this commitment, effective October 20, 2025, long-term care new business (LTC13 and LTC09M-CA) applications will be processed and administered directly by Mutual of Omaha.
To support the new business transition, new versions of our long-term care insurance applications are now available. Here are a couple of key enhancements to be aware of:
- Automatic Bank Payments for all modes (monthly, quarterly, semi-annually and annually) will now be available for selection at time of application, as well as after policies are issued.
- The option to choose between 1-5% compound inflation is now available on Custom Solutions in the state of Indiana, allowing clients greater flexibility in selecting the rate that best meets their needs.
Important Application Updates
Paper Applications
New paper application versions are available as of October 15, 2025 on normal ordering platforms.
- The national form number will change from MAP622_2024 to MAP622_2025. State-specific applications will also include the 2025 series number. New applications must be submitted to Mutual of Omaha starting October 20th, 2025.
Example: MAP622_NE_2025 (Nebraska). - Applications with a 2024 or prior series number received from October 20th through November 21, 2025 will be processed at Mutual of Omaha. After November 21, 2025 these application versions will not be accepted, no exceptions.
E-Applications
- The updated e-application is available on October 20, 2025.
- Applications started prior to October 20, 2025 can still be completed and submitted, but must be received no later than November 21, 2025, no exceptions.
