Previously, you were asked to provide both “Onset” and “Diagnosis” dates, which often meant relying on detailed records or an exceptional memory. Moving forward, all we need is:
- Medicare Name
- Prescribed Date
- Frequency / Dosage
- Diagnosis
We designed this enhancement to make completing a Short-Term application — especially when prescription medications are involved—more straightforward and less time-consuming for you.
