Long-Term Care Underwriting Enhancement: Statement of Good Health

Thrivent Agent with ClipboardWe are pleased to announce that effective immediately, Thrivent will no longer require a signed Statement of Good Health (SOGH) to be completed by the agent and client IF the outstanding requirements (including money) are received within 60 days of the Declaration of Insurability (DOI) being completed. The SOGH form will be required in the following circumstances:

  • If the Declaration of Insurability is greater than 60 days from date of underwriting decision
  • If the client(s) elect to increase coverage any time after the Declaration of Insurability is completed and before the policy goes inforce.
  • At the underwriters discretion an SOGH maybe requested, for example when the decision results in a class other than Preferred or Standard. Class 1 or Class 2 cases will require the SOGH to be signed by the agent and client as part of the policy delivery requirements.
