Reminder: Obtaining info on declined or rated cases
Get detailed insights on declined and rated LTC apps with new form
The Authorization to Release Underwriting information to Insurance Agent/Agency form (31368) is now available in the eApp.
This form, when signed by the client, allows you to obtain additional details from the underwriter about their decision in instances where a traditional, standalone LTC application is declined or rated. This form is only applicable if an application is declined or rated.
Even though this form is now available in the eApp, it can still be downloaded and attached manually if a particular client does not want to complete the form at the time of the application.
Attaching documents to Thrivent’s eApp
Attachments can be added to the application only after the application is locked and signatures are captured. There is space for up to four attachments and the total size cannot exceed 8MB.
NOTE: Only PDF files can be attached to an application.
- From the Left Navigation bar, select Attachments.
- Select an option from the drop-down list and click Attach PDF.
