We’ve made an important update to our HIPAA authorization forms for Fully Underwritten (FUW) Life applications.
What’s New:
An additional field has been added to capture the relationship to the insured when the application is signed by a parent or guardian.
Please begin using the updated HIPAA forms for all FUW Life submissions.
Note: The Drop Ticket process has not yet been updated to reflect this change. We will notify you once that update is complete.
